Open Start New Inspection, or scan an asset QR code. If that asset type has exactly one published template, the inspection starts with it; if several are published, you choose which one. See Conduct an inspection.

Organisation owners and administrators must use two-step verification (MFA) after a password sign-in. Your organisation can also require MFA for every password user in Organisation Settings → Security. Users do not turn MFA on or off from My account — setup happens at password sign-in when required. See Sign in and MFA. If you lost your device, ask another owner or admin to use Reset MFA on your user.

Asset QR codes open that asset for inspection (and can allow guest incident reports when an admin enables it). Incident-type QR codes always require sign-in. Details in QR codes and Incidents.

Yes. Install the app (PWA), then run Prepare for offline from your account before you lose signal. Sync status badges show what still needs to upload. See Using LambdaAssetCheck on a phone.

They are parallel work types, not stages of each other. Failed inspection answers can raise either, depending on the template. See Work management.

Access comes from roles (what they can do) and site assignments on the user (where they can see data). Groups do not grant site access. See Roles and permissions and Manage your users.
